The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
Medical treatment is available from the date of registration. Please contact reception for further information.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

If you wish to register you may print off a registration pack, fill out all the forms and bring them all in to the practice. Please also check with the practice before hand that you are within our practice area. We will be unable to register you if you live outside the practice area.
In addition to these forms you will need to bring along with you 2 pieces of identificaton. One will need to be a piece of photo identification (for example a driving licence or passport) and one will need to be something with your address on (for example a utility bill or official letter).
You will need to fill in one registration pack per person. Please choose from the Two packs below depending on the age of the person to be registered
Each Patient will also need to fill in a NHS GP registration form (also below)

We are unable to complete your registration until all forms are completed fully and we have seen identification .
We are able to offer on line access for repeat prescriptions, appointments and summary medical records. This service is known as Patient Access. (Note: For under 18s, only the repeat prescribing and appointments modules can be accessed).
To safeguard patient information, in order to register for these online services you will need to complete an application form and have your identity verified. This will also be the case for patients that are registered for on-line repeat prescriptions / appointments and wishing to access the other services.

PLEASE NOTE THAT ALL REPEAT PRESCRIPTIONS WILL NEED TO BE REQUESTED VIA A PATIENT ACCESS ACCOUNT FROM 1ST JULY 2015. THOSE CURRENTLY REGISTERED FOR REPEAT PRESCRIBING ARE KINDLY REQUESTED TO CREATE A NEW PATIENT ACCESS ACCOUNT IN ORDER TO ENSURE CONTINUITY OF THIS SERVICE.
